- Social Media Sharing: I consent to the use of my photographs and/or videos for sharing on NuHydration IV Therapy and Wellness' official social media platforms, including but not limited to Facebook, Instagram, Twitter, LinkedIn, and YouTube.

-Marketing Materials: I consent to the use of my photographs and/or videos for inclusion in NuHydration IV Therapy and Wellness' marketing materials, including brochures, flyers, posters, website content, newsletters, and other promotional materials.

-Media Coverage: I consent to the use of my photographs and/or videos for distribution to third-party media outlets, including newspapers, magazines, online publications, and television broadcasts, for the purpose of promoting NuHydration IV Therapy and Wellness.

I understand and agree that:

NuHydration IV Therapy and Wellness will have the right to crop, edit, alter, or enhance the photographs and/or videos as deemed necessary for promotional purposes.

I will not receive any financial compensation for the use of my photographs and/or videos.

The photographs and/or videos may be used indefinitely by NuHydration IV Therapy and Wellness and its affiliates, without any further approval from me.

NuHydration IV Therapy and Wellness will make reasonable efforts to ensure that my photographs and/or videos are used in a tasteful and respectful manner, in alignment with the brand's values and guidelines.

I acknowledge that I have read and fully understand the terms of this consent form, and I voluntarily agree to its contents.

 

This consent form is valid for all future photographs and/or videos taken of me during my participation in NuHydration IV Therapy and Wellness sessions, events, or activities, unless revoked by me in writing.
