  • Select your payment method and show the BCoBS volunteer the confirmation page for all online payments.*
  • Adopter must answer each question below in the affirmative (i.e. with a "Yes") to be approved for the Adoption of the Animal. Adopter acknowledges that the Adoption Fee is non-refundable.
  • Adopter represents and warrants that Adopter is adopting the Animal for Adopter’s personal companionship and not for any other individual or organization.
  • Adopter shall provide the Animal(s) with adequate care, attention, and affection, access to fresh food and water on a daily basis, shelter, training, grooming, clean litter, and exercise. If the Animal(s) adopted is a cat, Adopter shall home the Animal(s) as an indoor-only pet in a private residence.
  • Adopter agrees that they shall not abuse or physically harm the Animal. Adopter agrees never to have the Animal declawed, which permanently damages an Animal's ability to walk and may result in the Animal experiencing constant pain. Adopter understands that if BCoBS learns of any mistreatment of the Animal, BCoBS shall undertake all remedies to enforce this agreement, including removal of the Animal from the Adopter.
  • Adopter represents and warrants that the Animal will not be used for breeding, fighting, as a food source, or any form of profit.
  • Adopter acknowledges that the Adoption of the Animal is a commitment for the lifetime of the Animal, which could be 15 years or longer. Adopter further understands that if Adopter is unable or unwilling to care and provide for the Animal as agreed hereunder, the Adopter shall return the Animal to BCoBS with advance notice – no questions asked. In the event Adopter is unable or unwilling to care and provide for the Animal as agreed hereunder, the Adopter shall not surrender or relinquish the Animal to a humane society, shelter, or other person without first contacting BCoBS.
  • Adopter acknowledges that BCoBS makes no representations or warranties as to the health, temperament, habits, or background of the Animal. BCoBS will provide all veterinarian records in its possession. Adopter acknowledges that an underlying condition may exist about which BCoBS is not aware at the time of the adoption, and for which BCoBS is not responsible.
  • FOR ADOPTERS WHO ARE ADOPTING ANIMALS UNDER SIX MONTHS OF AGE ONLY. Spaying or neutering is required by six (6) months of age. BCoBS can provide assistance for scheduling this procedure, which is covered by the Adoption Fee. If the Animal is not spayed or neutered prior to Adoption or through BCoBS-approved providers, then Adopter shall provide proof of spaying or neutering to BCoBS within ten (10) days after such procedure.
  • Adopter acknowledges that Adopter is solely responsible - legally and financially - for the Animal from the date of Adoption forward. This includes, but is not limited to, providing access to fresh food and water on a daily basis; shelter; grooming; clean litter; and proper veterinary care. Proper veterinary care includes regular wellness visits, vaccinations, as well as any necessary emergency care due to illness or injury. BCoBS is not responsible, financially or otherwise, for any medical problems that are discovered or diagnosed after the date of Adoption, nor is it responsible for any expenses or liabilities arising from or related to the Animal that are discovered or incurred after the date of the Adoption.
  • Adopter's contact information will be assigned to the Animal's microchip upon completion of this agreement. BCoBS will provide accurate chip information to Adopter, who will be responsible for registering chip.
