  • Cancellation, No Call No Show, and Rescheduling Policy

    If you are unable to attend your scheduled appointment, please provide at least 24-hour notice. Cancellations on the day of service will incur a 50% cancellation fee, charged to the card on file. This fee may be waived if you reschedule within 48 hours of cancellation, regardless of when the new appointment takes place. Appointments can only be rescheduled once. If the rescheduled appointment is cancelled, 100% of the original service cost will be charged. If no card is on file, the fee will be added to your total at your next service. No Call-No Shows will result in a charge of 100% of the scheduled service. A second No Call-No Show will require pre-payment for future services or may result in the inability to receive Skinvestment by Haley services.   • Informed Consent

    Although every precaution will be taken to ensure your safety and wellbeing before, during and after your spray tanning treatment, please be aware of the following information and possible risks. Please read the following carefully:

  • The spray tanning service will take place in a private room with a changing suite where you may undress to your level of comfort; undergarments (bottoms) must be worn at all times. Disposable undergarments and pasties are provided and available for your use at your discretion.

    The spray tanning process will take approximately fifteen to fifty minutes, depending on the scheduled service(s). After spraying, your skin should remain dry until it is time to rinse off, following the advised time given by the artist.

    The solution provides an immediate bronzing effect from cosmetic bronzer. The bronzing effect is a result of a temporary coloring additive in the solution that will remain on the skin until the first shower and may persist if proper care is not followed. Bronzer may transfer to clothing, furniture, car seats, or bedding. Skinvestment by Haley cannot and does not guarantee that product transfer will not occur, as spray tans are a full-body cosmetic service. It is suggested and best practice to wear loose, dark clothing, specifically long sleeves and long pants, to your appointment and to remain in such clothing until your initial rinse. After rinsing, clothing should not be tight-fitting or textured to avoid product transfer and uneven color development. If you arrive in shorts, it is recommended to bring a towel to cover car seat(s) to avoid bronzer coming into contact with upholstery. Upon showering, the majority of bronzer will rinse off to reveal the tan beneath. The skin will appear lighter at first, but over the next 24 hours, the tan will continue to develop to its final tone. Please note that Skinvestment by Haley is not liable for any cleaning, replacement, or corrective actions regarding items where product transfer has occurred.

    All ingredients used in this procedure are intended for cosmetic use and are generally regarded as safe. However, individuals may be allergic to one or more ingredients in the spray tan solution. If you have any known allergies, please inform the artist in the space provided on this form. A full ingredient list is available upon request.

    By signing below, you consent to the following:

    I understand that spray tanning is achieved through the application of a solution containing Dihydroxyacetone (DHA), which is FDA-approved for external use only.
    I acknowledge that I have the right to request nose plugs, protective eyewear, and occlusive lip balm to protect my mucous membranes from DHA exposure, in line with FDA recommendations.


    I understand that the spray tan does not contain sunscreen (SPF) and does not protect against sunburn. I acknowledge that spray tanning over a current or recent sunburn (within 1-10 weeks) may result in undesirable outcomes, and the artist may refuse to spray over sunburned areas or decline the service altogether.


    I understand that spray tanning may aggravate certain conditions such as allergies, asthma, and other respiratory conditions, and I consent to proceed with the service should I have any of these conditions.


    I understand that it is not advised to receive a spray tan for the first time when my appearance is critical (wedding/special occasion) due to the potential for unfavorable results. Bridal clients must schedule a trial tan at least 2-3 weeks prior to the wedding. Week-of services will not be accepted.


    I understand that if I am undergoing chemotherapy, I must consult my provider for permission prior to the appointment and explicitly inform the spray tan technician of the approval. Should I choose not to consult my provider, I accept full responsibility for any reactions or adverse results from receiving the spray tan and release Skinvestment by Haley from any liability.

    I understand that while reactions are rare, they may include itching, redness, rash, coughing, dizziness, fainting, or other irritations, and that Skinvestment by Haley is not liable for reactions arising from the service.


    I confirm that I have read, understand, and will follow the prep care instructions provided via text message or Skinvestment by Haley’s business page(s). I understand that factors such as skin conditions, exfoliation practices, products used, compliance with prep care, lifestyle, season, post-service activities, and more influence the final result and longevity of the tan. Skinvestment by Haley is not responsible for results impacted by these factors.


    I understand that, prior to the appointment, my skin should be exfoliated and free from lotions, perfumes, deodorants, and makeup, and that some products and medications may interfere with the development of my tan. If I arrive without following these steps, I acknowledge that the quality of my spray tan is my responsibility, and Skinvestment by Haley is not liable.

     

    I understand that once I leave the appointment, maintaining the tan is my responsibility, and Skinvestment by Haley is not liable for any issues related to improper care, exposure to water, sweating, or other activities that may affect the longevity and quality of the tan.


    I understand that product transfer may occur and accept full responsibility for any transfer that occurs on clothing, furniture, or personal belongings. Skinvestment by Haley is not responsible for correcting any transfer issues.


    I affirm that, to the best of my knowledge, I have no medical condition or allergies that would prevent me from receiving a spray tan, and I accept responsibility for any reactions or staining of personal items.


    If I am the legal guardian of the individual receiving this service, I understand, agree, and consent to all statements above as outlined in this consent form, and agree to the terms of the cancellation and rescheduling policy.
