The standard Medical Form for all Junior Rugby League players is designed to assist volunteers to care for the children and young people registered to play during the season. This form is not meant to be in any way an invasion of the privacy of the players, or their families, nor will it be given to any other person unless they give permission for this to happen.
The only people who will access this form will be the appointed Rugby League First Aid Officer, the Team Manager and/or the Team Coach, the child's private doctor and/or qualified personnel employed by the Queensland Ambulance Service (QAS) who may be given this information to support the care and medical treatment of players in the case of a serious injury.
This private information is entrusted to the Club in order to maintain the safety of its players.

I acknowledge that the information contained on the Medical Forms required by Edmonton Junior Rugby League and its governing organisations is only to be used for the safe treatment of players in the event of a medical incident during official training sessions and sanctioned games.
I declare that the information contained on the Medical Form will only be accessed by authorised members of the Club to cater for the specific medical needs of the players. I acknowledge that I am responsible for maintaining the privacy and confidentiality of this information and I will keep the forms secure for the duration of my role at the Club.
I will delete and/or return the folder with ALL Medical Forms enclosed at the end of the season. I will NOT make copies or take photos of these documents and will not share in any format or on any social media platform.
