     I hereby acknowledge that I am voluntarily participating in sports activities organized by the HAYSB (HURLEY AREA YOUTH SOFTBALL & BASEBALL). I understand that these activities involve inherent risks, including but not limited to bodily injury, property damage, and death, which may occur as a result of my participation. Therefore, I agree to the following terms:

    RELEASE AND WAIVER OF LIABILITY: I hereby release and forever discharge the HAYSB organization, its directors, officers, agents, and volunteers (collectively, the "Released Parties"), from any and all liability, claims, demands, and causes of action that I may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise.


    ASSUMPTION OF RISK: I understand and acknowledge that my participation in the sports activities organized by the HAYSB organization involves inherent risks, including but not limited to bodily injury, property damage, and death. I voluntarily assume all risks associated with my participation in these activities.


    INDEMNIFICATION: I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, demands, and causes of action, including attorneys' fees and costs, arising out of or related to my participation in the sports activities organized by the HAYSB organization.


    MEDICAL TREATMENT: I authorize the HAYSB organization, its agents, and volunteers, to obtain or provide medical treatment for my child in the event of any injury, illness, or other medical condition that may occur during my participation in the sports activities.


    ACKNOWLEDGEMENT OF UNDERSTANDING: I have read this sports waiver, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue the Released Parties for any injury, death, or damages that may result from my participation in the sports activities organized by the HAYSB organization.

  • Concussion Information

  • The Wisconsin law called the “Sidelined for Safety Act” is found at Section 118.293 of the Wisconsin Statutes (W.S.A. Section 118.293). This law is under the Public Instruction statutes and General School Operations. 

    "The Wisconsin law requires the Department of Public Instruction and the Wisconsin Interscholastic Association to develop guidelines to inform and educate pupil athletes, their parents or guardians, and coaches of the nature and risk of concussion in youth athletic activities. The statute defines “youth athletic activity” as an organized athletic activity in which participants a majority of whom are under nineteen (19) years of age, are engaged in an athletic game or competition or in a practice or preparation for an organized athletic game or competition against another team, club, or entity. Youth athletic activity does not include college or university activity or an activity that is incidental to a nonathletic program. It requires a person operating a youth athletic league to distribute a concussion and head injury information sheet annually to each coach and to each participating athlete. The information sheet for each youth athlete shall be signed by a parent or guardian and returned prior to participation."

    Concussion forms will be distributed to parents/players and must to signed and returned to before participation in practice or games.

    Who - 3rd to 6th Graders

    When - Saturdays – April 20, 27 & May 11, 18

    Boys 8:00 am – 10:00 am

    Girls 10:00 am – 12:00 pm

    Where - Hurley School or HAYSB Fields (weather permitting)

    Camp Cost  - $50/camper

     

    Registration flyers will be sent home with students or email us for a copy at hurleyareayouthsb@gmail.com! Forms and payment will be due by March 18th.
