I know this seems super annoying, but my business is all about creating relationships. I get to know my models well, so it's super important for me to have as much info about you up front as possible! Don't stress about the answers - just answer these in your own voice, so I can get to know you better :)

4. You must have your parent/guardian's permission to fill out this form. I will contact them once I receive your application to talk with them about the Senior Model Rep program
