Tattoo Appointment Request Form
Thank you for your interest in getting tattooed by me! Please fill out the questionnaire below to help streamline the booking process. If there are any question you are unsure how to answer, fill them out to the best of your ability and we can discuss further in our follow up conversations. Keep in mind I only tattoo people who are 18+
Booking Policy
Please note this does not guarantee your appointment. Upon booking I require a non-refundable deposit that will confirm your appointment time and be applied to your final session. Deposits will be $50-$100 depending on the size of tattoo. I require a 72 hour notice for any changes to the design including color palette. For rescheduling I require a 48 hour notice (2 days before you appointment) and an exact date for us to change the appointment to. You will be able to reschedule the appointment twice before your deposit is forfeited and a new one is required to move forward. If you cancel or are a no call/ no show your deposit will be forfeited. If you have a budget I am happy to work around it if possible. Keep in mind that doesn’t guarantee your desired piece can be executed under a low budget. After you fill out the below questions I will have a better idea of exactly what your looking for and will be in contact with you to get you officially booked! Please allow 3-5 business days for a response. I look forward to hearing from you and bringing your ideas to life!
