  • In order to properly prepare your return all the relevent information needs to be provided:

    • If you are a new client, provide copies of last year’s tax returns.

    • All Income forms, such as W-2, 1099-INT, 1099-DIV, 1099-B, 1099-R, Schedules K-1s, and other income reporting statements.
    • Form 1095s for health insurance.

    • Business Income and Expense information such as profit/loss, income and expenses categorized, bank statements, etc.
    • Logs for Business Use of Vehicle including total miles, business miles, personal miles, and commuting miles.
    • If you bought or sold real estate, then we need the closing statements.

    • Any estimated tax payment information.
    • Any information regarding medical, interest, or miscellanous tax deductions.
    • Detailed information for charitiable contributions.

  • You agree to furnish us with all information regarding income and deductible expenses. In the event that you receive additional information after we initiate the work on your return, you commit to promptly contacting us to ensure the inclusion of all pertinent details in your completed tax returns. You confirm the accuracy of all expenses or other deduction amounts and assert that you possess all necessary supporting written records. There may be instances where we request to review your documentation, and you must be prepared to provide written records for all items on your return if subject to an audit by either the IRS or state tax authority. We can offer guidance on acceptable evidence.

    By signing below you are stating:
    "I attest to the authenticity and precision of the expenses and income outlined above, confirming that I, either as the taxpayer or spouse, have personally submitted them to the tax preparer. I acknowledge my responsibility to furnish comprehensive, accurate, and truthful information to the tax preparer, and I absolve the tax preparer of any liability for inaccuracies, including but not limited to audits resulting from the provided information."

    Before affixing your signature, thoroughly review the return to verify the accuracy of the information. Fees must be settled before the delivery or filing of your tax return. Should you terminate this engagement before completion, you agree to remit a fee for the work already performed. A retainer is mandatory for the preparation of overdue returns.

    It is advisable to retain a copy of your tax return and any associated tax documents. A fee may be imposed if you request a copy in the future.

    Also, by signing below, you acknowledge that you have read, understood, and accepted your obligations and responsibilities.  

  • Tax Preparation Liability

  • We highly value our client relationships and are dedicated to providing you with the best possible service. We want to remind you that you are not being provided a tax refund, but instead a correct and accurate tax return based on the information you provided.

    We will compile your tax return using the details you furnish. In the event of an audit, you are accountable for validating the reported items. Prior to signing, please thoroughly examine the return to confirm the accuracy of the information. Unless explicitly specified, the services for crafting your return do not encompass auditing, review, or any other means of verification or assurance.

    Audit Maintenance Pro may be offered as an add on to your return. Please speak to your tax preparer for more details. 

  • Automated Clearing House Authorization

  • I hereby authorize MRP Bookkeeping LLC to initiate Automated Clearing House (ACH) transactions to debit my account for the purpose of paying tax preparation fees associated with my tax return.

    By providing this authorization, I understand and agree to the following terms:

    1. I authorize MRP Bookkeeping LLC to debit my account for the total tax preparation fees as agreed upon for the services rendered.
    2. This authorization is valid for the current tax year and any subsequent tax years for which I engage MRP Bookkeeping LLC for tax preparation services.
    3. I understand that the ACH transaction may take a few business days to process, and I must ensure that my account has sufficient funds to cover the debit.
    4. In the event that the ACH transaction is unsuccessful due to insufficient funds or any other reason, I agree to promptly provide an alternative payment method or settle the outstanding amount.
    5. I have the right to revoke this authorization at any time by providing written notice to MRP Bookkeeping LLC, but such revocation will not affect transactions already initiated.

    I understand that this authorization will remain in effect until MRP Bookkeeping LLC has received written notification of its termination or until the completion of the tax preparation services.
