Informed Consent, Acknowledgement & Release of Liability

I hereby give my approval for my child’s participation in any and all activities of Frenji Sports Academy, during the selected session. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Frenji Sports Academy, TDYCC and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of participating in Frenji Sports Academy during the season.

There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

In case of injury to said child, I hereby waive all claims against Frenji Sports Academy, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.

I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 is a possibilty. Frenji Sports Academy and TDYCC have implemented preventative measures according to the most recent CDC policies. I assume all risk and hold harmless Frenji Sports Academy, TDYCC and all its respective officers, agents, and representatives from any and all liability for the spread of infection.

Furthermore, all images and videos of Frenji Sports Academy are the property of Frenji Sports Academy and can be used for any promotional consideration.

I hereby state that I have carefully read the above waiver. Acceptance and understanding of this agreement are hereby acknowledged. I have read and agree to the Informed Consent, Acknowledgement and Release of Liability.
