Welcome to Summer Camp at Montessori on the Lake!

2024 Reservation, Agreement, Permissions, and Waivers
June 17-August 23*

For students age 5 (who have completed kindergarten) through 12.

$350 per week, 7:00am-4:00pm. Additional $50 per week for extended afternoon care 4:00pm-6:00pm.

*Week 10 (August 19th) is available exclusively for students enrolled in Montessori on the Lake or Sunflower Montessori in grades 1st - 8th for the 2024-25 school year.
We are so happy to have you here for the summer at Montessori on the Lake. Thank you for taking the time to fill out this required information. You may find it helpful to have the following details on hand while completing this form: contact information (phone numbers, addresses, plan numbers, etc.) for emergency contacts, and your child's medical/dental providers.

Upon submitting this form you will be charged 50% of the total owed through your Procare account. The remaining balance will be charged on July 1st 2024. Registration is on a first come basis.
