The Community Discipleship Partner Form serves as a tool to connect individuals with the discipleship pathways within our community. Through this form, we aim to:

1. Identify New Discipleship Partners: Help partners who or interested individuals formally express their desire to join the discipleship community

2. Facilitate Growth and Engagement: Understand each person’s spiritual needs, interests, and goals so we can guide them to appropriate programs, mentorship, or support.

3. Build Connections: Provide a pathway for building meaningful relationships within the community, fostering accountability, encouragement, and fellowship.

4. Support Spiritual Development: Ensure that each person has access to resources, teachings, and support that align with their journey in faith.

This form enables our Hub to organize discipleship activities, support individual growth, and create a welcoming space for everyone seeking a deeper connection with God and the community.
