Participant Declaration

To participate in our activities, you are agreeing to the following conditions.

•        I recognise that adventurous sports can be dangerous and I/the child in my care will ensure I/they understand the need for safe and responsible conduct and behaviour to ensure everyone has a safe and enjoyable time.

•        Participants must not suffer from any undisclosed medical or physical condition which might endanger themselves or others.

•        For participants under 18 years, a parent or guardian must sign this form and in doing so authorises ADVENTURE 4 YOU staff to approve emergency medical treatment should the need arise.

•        I agree that ADVENTURE 4 YOU staff may photograph or video those taking part in any ADVENTURE 4 YOU activity and may use or store this media.

•        ADVENTURE 4 YOU will endeavour to provide you with an enjoyable and worthwhile activity day although there may be occasions when we cannot complete the whole programme, for example strong winds, poor weather or other conditions dictate. In such cases, we will alter programme.

•        I agree the person I am booking for will behave in an appropriate manner and listen to all instructions given by the ADVENTURE 4 YOU staff. I understand that failure to comply with ADVENTURE 4 YOU staff may lead to the person being unable to participate in activities and may result in a parent/guardian being called to collect them early.

•        By completing this form, you are consenting to ADVENTURE 4 YOU storing and retaining your data in accordance to the GDPR requirements. This may include passing your data to a secure third party to protect service users, staff and ADVENTURE 4 YOU as required, as part of meeting health and safety requirements.

•        I agree that I am responsible for any costs incurred by ADVENTURE 4 YOU as a result of non-accidental damage to equipment or property by myself/ the person for whom I am a legal guardian.

•        I agree ADVENTURE 4 YOU are not responsible for any valuables myself or the person for whom I am a legal guardian for bring to site.
