Consent and Release of Liability Form

I willingly participate in Sound Alchemy, a service by Matthew Peters, involving sound therapy with singing bowls to enhance well-being. Understanding that Matthew Peters is not a medical professional and this is not a substitute for medical treatment, I acknowledge the non-invasive nature of sound therapy. I'm aware that emotional release may occur. I consent to the placement of singing bowls on and around my body, understanding this aids relaxation and balances energy centers. In consideration for the service, I agree to voluntary participation, informed consent, and release Matthew Peters and Sound Alchemy from liability. I assume associated risks, disclose medical conditions, and understand the confidentiality of shared information.I am aware that I have the right to stop the session at any time. Additionally, I acknowledge and consent to the possibility of gentle and non-intrusive touch by the practitioner during the sound bath session. This touch may involve placing hands lightly on specific areas of the body or providing gentle support as needed to enhance the therapeutic experience. The practitioner will always prioritize comfort and respect personal boundaries. If, at any time, I feel uncomfortable with the level or type of touch, I understand that I have the right to communicate my preferences or stop the session. By signing, I confirm understanding and agreement as confirm that I am at minimum 18 years of age.

In full consideration of the risk of injury while participating in the activity of Sound Bath, and for the right to participate in the activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating in Sound Baths, which may include, but are not limited to, physical or phycological injury, pain, suffering, illness disfigurement, temporary or permanent disability, economic or emotional loss, and death.

I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
