Before booking your session with Mya, please review and agree to the following waiver and consent below:*

I acknowledge that Mya of Nailz By Mya is a nail artist, please be aware that Mya specializes in nail artistry, focusing on creative designs while maintaining general clean practice standards during services.By booking, I acknowledge and relinquish Mya from any liability or harm arising from the services provided by her. For specific nail health concerns, Nailz By Mya recommends consulting a certified nail technician.I confirm that the nail artist will not be responsible or liable if the result of the service is not as expected as it should be.I am allowing Mya, a nail artist, to apply the necessary chemicals as part of the service in my nail application.I agree that the nail service is final after the service. If there are any changes after 1 hour when the service ends, the client will be charged for any modifications.I understand that extra guests that have not been previously communicated with Mya are not allowed in the work service area for safety reasons.I have read this consent and waiver, and I accept the terms indicated above.
By signing below, I confirm that I have provided accurate and current information on this form and I agree that I have made this consent and waiver voluntarily. In any case that I decide to withdraw or revoke my waiver, I may do so by submitting a written request signed by me to NailzByMya.
