Yes No Conditions of membership I/We acknowledge that E DIMOSKA & Z DIMOSKI trading as RONIN MAMISDE KARATE ("RMK") have the right to determine membership at its absolute discretion .E DIMOSKA & Z DIMOSKI trading as RONIN MAMISDE KARATE ("RMK") have worked out the annual price for training and divided it into 26 easy payments. RMK will not stop your payment over Christmas and school breaks. I/ We acknowledge that fortnightly training fees must be paid on time and keept to date all times I/We acknowledge that fortnight training fees will still be paid over the Chirstmas and school term breaks. I/We acknowledge that fortnightly payments, we must let RMK know in writing with a minimum notice provision of 4 weeks from the date that written notice is recived by RMK. I am aware that the nature of training in Martial Art and KARATE as any Martial Art and as a sport, and recreational activity, has an inherent risk of injuries and that by participating I am exposed to certain risks. The type of injury which can occur includes soft tissue ,orthopedics(bone) ,neurological, spinal injuries among others injuries. acknowledge and understand that whilist participating in Martial Arts. I may be injured ,other persons participating in Martial art may cause me injury or may damage my property, or I may cause injury to other persons or damage their property. -| acknowledge and understand that before, during and after training my child and I must act and obey in accordance with curent Government recommendations regarding to Corona Covid 19. For more Information about Coronavirus visit health.gov.au. -| assume the risk of and responsibility for any injury,death or property damage resulting from my participation in Martial Arts. I am aware of possibillity of these injuries and in joining a training session( in allowing a child under my supervision to participate in a training session) I take full responsibilty for this decision, further I am cognisnat of the consequences of this decision.

Training with E DIMOSKA & Z DIMOSKI trading as RONIN MAMISDE KARATE ("RMK") I (on my own behalf and/or on behalf of the child hereby release RMK, its instructors, officers and administrators to the maximum extent permited by the law, from: a All responsibilityy or liability from any act, omission, or neglect causing loss or damage to my self and/or child, personal property or injury to my self and/or child or death to my self/or child. b All claims, suits, actions or proceedings or demands,costs,charges and liabilities now or in the future or whatsoever nature, araising out of or in any manner whatsoever connected to myself and/or child with participation in and activity with RMK. c from the date myself and/or child first participates onwards. I confirm I have legal care and control of {firstName} and request the child is accepted as a member of RMK. Iagree to ensure that before training myself and/or child will remove any watches, jewellery or other items which might cause injury to another person and will ensure myself and/or childs have closely trimmed toe and fingernails, and in good health with no infections diseases. I agree to ensure at all times I and/or child are equipped with appropriate protection equipment for the acivity being conducted for the safety for myself and or/ child and others. Without limiting the operation paragraph below, acknowledge on my own behalf and on behalf of the child that the intended effect of the paragraph below ,I acknowledge on my own behalf and on behalf of the child that the intended effect of the paragraph below is to relive RMK, its instructors, officers and administrators, to the maximum extent permitted by law, from any liability or responsibility it(they) may otherwise have had arising out of or connected with the Childs participation in RMK aactivities wheter on the date the child first participates in RMK activities or after that date.

I acknowledge on my own behalf of the child that this document, may, to the maximum permitted by law, be pleaded in any court as a bar to any action, suit, claim, cause or proceedings commenced or at any time to be commenced by me, the child, or any person claiming through either of us against arising out of or in any manner whatsoever connected with the Childs participation in RMK activities. I unconditionally and irrevocably agree to indemnify the RMK, its instructors, officers and administrators, for any loss, liability, cost, charge or expense, claim, suit, action or proceedings or demand now or in the future of whatsoever nature suffered or incurred by RMK activities, whether on the date which the child first participated in the RMK activity

The Student hereby represents that he/she is physically fit to receive and participate in the prescribed course of Instruction, and will faithfully comply with all the rules and regulations of the School and the traditions of the Martial Arts. I further understand that as the Payer I am purchasing enrolment at this School for the student listed of this Agreement. Enrolment provides the students with the opportunity to attend the specified number of classes. Failure of the student to attend the appropriate number of classes does ot relieve me of my obligation to pay for their enrolment in full. My payment schedule is not affected b the lesson schedule and/or attendance of the students listed on this Agreement. I understand and agree that the school will not be held liable for injuries, damages etc not caused by or resulting from the negligence of the owners, operators, employees or persons in charge of

According to new regulations reqested by AKF every year every student member of RMK must renew and pay annual membership fee of 27.50$ via karateaustralia.org.au/membership or 30$ via karate club (27.50 +2.5 )admin fee. The membership fee includes AKF member services and comprehensive insurance cover, Member Organisation Insurance -Public Liability/profesional Indemnity,Access to State, National, Oceania Championships, Commonwealth WKF Premier League, Series A and World Championships

I understand and agree that training sessions fees will be payed per forthnight FFA PaySmart or the School from my account. I accept that payments presented against my account that are returned unpaid will be automatically re processed prior to the next instalment due date. Payments still unsuccessful by this time will be processed with the nest instalment on that due date. I accept that all and any costs of collection incurred by FFA PaySmart or the School or their representatives, including (without limitation) collection agency cost, court costs, solicitors fees on a solicitor/client basis, together with interest on the outstanding amounts at the rate allowed by the court from time to time and all reasonable additional costs of recovery of the amount outstanding will be charged to the Payer. I acknowledge receiving a copy of this agreement.

1. You may cancel this agreement without penalty or further obligation within 7 (seven) days from the date of this Notice of cancellation shall be in writing and mail to the school by registered or certified mail. Down payments shall be fully refunded minus the cost of any lessons taken within the 7 days and the cost of any merchandise included in the course price (e.g. Uniform 2. Applications to suspend this Agreement will be considered according to the specific circumstances below. The suspended period can not be during school holidays and must be in multiples of a full calendar month, for a minimum of 1 (one) month and a maximum of 3 (three) months. The Payer's installment payments must be current and up to date. Applications to suspend this agreement must be submitted in writing to the School prior to the student/s' absence. If approved, the suspended period (included all payments scheduled within this time) will be added to the previous completion date of the Agreement. Approval is the sole discretion of RMK and FFA PaySmart. 3 Application for early Termination of this Agreement will be consideret according to specific circumstances below. The Payer's instaltment payments must be curent and up to date prior to submitting your application. All applications for early Termination must be in writing, and must be submitted to the School giving at least 14fourteen) days notice. Note that payments scheduled within this period will still be due. 4 The school's fees have structured in closure during the Christmas Holiday and School Holiday periods, and during the national karate Championships however if the school is closed for a month or more outside these times, you are entitled to your choice either an extension of the agreement or prorated refund, except if the closing is not the fault of the facility, in which case the choice of remedy is the School's. PHOTO, VIODEO, AUDIO RELEASE. You agree that may take photographs and may take video and audio material of Student's classes and schools events, and that these materials may be used for display, promotion and/or advertising, or sold for profit, and the student hereby waives any compensation to which they may otherwise be entitled for appearing in such materials. 6 Both FFA PaySmart and the School understand the importance of, and are committed to complying with, the Privacy Act (1988) and the National Privacy Principles. We only collect information that is necessary for us to effectively manage your tuition payments and provide you with Martial Arts lessons. In the event of non-payment we may contract out services that involve disclosing your information to other organizations (including legal advisors and collection agents 7 This Notice of Costumer's rights are an integral part of the Application and Agreement for Membership.

If you or your child has any disadvantaged that may affect their performance at a grad.ng we should be advised. It will be taken into considiration by the examiners at the grading.

Ensure that when you train you do so at a pace that you are comfortable with and if you experience any difficulties make sure to inform instructor. Should you suffer any injury, illness or condition in the future, please inform us Satatment: I recognise that the RONIN MAMISDE KARATE its instructors, ,officers,administrators, not able to provide me with medical advise with regard to my fitness and/or medical conditions and that the above information is true and complete in all extents and will be treated with confidentiality.
