                                      • TERMS AND CONDITIONS:

                                        The following terms and conditions shall form part of this application and once signed will be part of any rental contract or agreement resulting from the acceptance of this application by All Saints’ Anglican/First UnitedChurch or its authorized representative. The applicant, acting on behalf of the organization, will be responsible for the following terms and conditions: 

                                        1. Rental will be confirmed ONLY upon receipt of application form, insurance documents, and payment in full. A written confirmation will be sent from the facilities rental manager or Parish Office.
                                        2. For ‘session’ rentals that span multiple dates, one (1) month notice is required for cancellation. In lieu of such notice, a fee equivalent to one month’s sessions is levied.
                                        3. For single event rentals, a cancellation requires a notice of ten (10) business days in advance to the event in order to receive a full refund. In lieu of such notice, no refund will be possible.
                                        4. Any misconduct or damage; food & beverage if required; liquor permit if applicable; leaving kitchen and other areas used clean and as they were found; all rented items including dishes and utensils will be returned to designated locations, appropriately cleaned and carefully handled.
                                        5. The applicant hereby agrees to provide a certificate of Third Party Liability in the amount of two (2) million dollars and shall assume all  responsibility for all injuries, or damage to persons or property of the applicant, its’ members, agents or invitees arising out of or in connection with the rental and the use of Church buildings and lands and shall indemnify All Saints’ Anglican/First United Church from and against any and all claims for such injuries, loss or damage. A copy of this certificate is needed to be submitted before the application will be confirmed.
                                        6. When alcoholic beverages are being served, the applicant or representative (NOT ASW) shall ensure that a Liquor Licence is obtained and displayed, and the age of majority rules shall apply and be strictly adhered to. A copy of the license needs to be submitted 2 business days before the event. It is the responsibility of the Applicant to follow all rules and instructions from the Alcohol and Gaming Commission of Ontario website (https://www.agco.ca/alcohol/special-occasion-permits-private-event). Applications for a ‘Private’ Special Event permit requires an account on iAGCO (https://agco.ca/iagco).
                                        7. The applicant shall ensure that only the designated area(s) plus washrooms, entrances, and landings shall be used and that access to other areas of the building is not permitted.
                                        8. This is a NO SMOKING  building - the applicant (NOT ASW) shall be responsible for ensuring the NO SMOKING Policy is enforced. Smoking can take place 9 feet away from any building entrances.
                                        9. Any forms of confetti, rice, or bubbles are not allowed in the building. 
                                        10. The function or activity may be monitored by the Rental Agent or designate at any time without notice.
                                        11. As per The Anglican Diocese of Ottawa’s COVID-19 plan (In This Together, ver 4 - 2022/05/25), level GREEN mask wearing is
                                          recommended (but not required) while in the building for large gathering where social distancing is not possible.
