Online Consent and Agreement Form for Adults

By checking the "Agree" box below, I, the undersigned participant ("Participant"), hereby consent and agree to the following terms outlined in this agreement:

I hereby consent and agree that VRTU - Virtual Reality Therapy Unit Pty Ltd's member has the right to take or use photographs or record video of me (and/or my property) and to use these in any media form worldwide including online (Website sites and social platforms)  now or hereafter known, and for any purpose whatsoever. 

 

Participation Consent with Compensation Options: I acknowledge and grant permission for my participation in the production of specific audio and/or visual content, understanding that this participation may be on a voluntary (unpaid) basis or may involve financial compensation. This consent extends to my identification in the resulting content across various media formats, including, but not limited to, digital and analog photography, film and videography (with or without sound), and audio recordings. The content may be produced or captured either directly by the party referred to as "Releasee" or under their direction.

Transfer of Rights: I irrevocably assign and transfer any and all current and future financial interests, as well as any physical or intellectual property rights in the content, to the Releasee, along with their heirs, successors, and assigns. This transfer grants them the complete liberty to utilize the content for any lawful purposes without requiring further authorization from me or the provision of additional compensation, irrespective of the nature of my participation.

Authorization, Waiver, and Ethical Assurance: I authorize the unrestricted use and reproduction of my voice and/or likeness in connection with the content by the Releasee and their authorized agents. This permission applies to all forms of media, both existing and yet to be developed, for any purpose, including but not limited to streaming, film, television, advertising, art, promotion, and commerce. I waive any right to review or approve any application of the content or its utilization. Furthermore, I affirm that no sexual activity has been or will be involved in any aspect of the content's production, maintaining the highest ethical standards. I agree to indemnify and hold harmless the Releasee and their successors from any claims arising out of the content's use or any breach of this affirmation.

Complete Agreement Acknowledgment: This document represents the full agreement between the Releasee and me, governed by the laws of the specified jurisdiction. By selecting "Agree," I affirm that I am of legal age and have the full legal capacity to enter into this agreement. I have read, fully understood, and agree to the rights and obligations set forth herein for all parties involved.

By marking the "Agree" checkbox below, I, the undersigned ("Releasor"), in my capacity as the parent or legal guardian of the minor herein referred to ("Minor"), hereby consent and agree to the following terms under this agreement:

Participation Consent with Compensation Options: I hereby grant permission for the Minor to participate in the production of specific audio and/or visual content, acknowledging that this participation may occur on a voluntary (unpaid) basis or may involve financial compensation. This consent encompasses the Minor's identification in the resulting content across a variety of media formats, including, but not limited to, digital and analog photography, film and videography (with or without sound), and audio recordings. This content may be produced or captured either directly by the party known as "Releasee" or under their supervision.

Rights Release: I irrevocably assign and transfer any current and prospective financial interests, along with any physical or intellectual property rights of the content, to the Releasee, as well as their heirs, successors, and assigns. This assignment allows them unfettered freedom to use the content for any lawful purpose without the necessity of obtaining further permission from me or providing additional remuneration, regardless of whether the Minor's participation is unpaid or compensated.

Authorization, Waiver, and Ethical Assurance: I authorize the unrestricted use and reproduction of the Minor’s voice and/or likeness in relation to the content by the Releasee and their authorized representatives. This authorization applies to all media forms, existing and yet to be developed, for any purposes, including but not limited to, streaming, film, television, advertising, art, promotion, and commerce. I waive any right to inspect or approve any application of the content or its use. Additionally, I affirm that no sexual activity has been or will be involved in any aspect of the content's production, upholding the highest ethical standards. I agree to defend and hold harmless the Releasee and their successors against any claims stemming from the content's use or any breach of this affirmation.

Comprehensive Agreement Acknowledgment: This document constitutes the entire agreement between the Releasee (VRTU - Virtual Reality Therapy Unit Pty Ltd and me, governed by the laws of the Victoria state. By selecting "Agree," I declare that I am legal of age and possess the authority to make this agreement on behalf of the Minor. I acknowledge having read, understood and agreed to the responsibilities and rights delineated herein for all parties involved.
