  • Is your agency a Victims Service Provider? A VSP is defined as a private nonprofit organization whose primary mission is to provide services to victims of domestic violence, dating violence, sexual assault, or stalking. Such term includes rape crisis centers, battered women’s shelters, domestic violence transitional housing programs, and other programs.*
  • If you have identified as a VSP, your agency is prohibited by HUD to use HMIS. You must use a comparable database.  You will not be able to fill out the rest of this form.

  • Does your agency have experience operating homeless services programs?
  • Does your agency receive CoC or Federal funding?*
  • Is your funder requiring you to use Coordinated Entry?*
  • Would you like access to Coordinated Entry (aka OneHome) to complete CE Assessments (formerly known as the VI-SPDAT)?*
  • What type of HMIS access are you interested in (this can always be changed)*
  • Do you know what type(s) of project(s) you're being funded for?
  • Is your agency led by those who identify as BIPOC (Black, Indigenous and People of Color) or those with Lived Experience of homelessness?*
  • Do 50% or more of the clients you serve identify as BIPOC (Black, Indigenous and People of Color)?*
