You need to hang a chain fall overhead. The rolling scaffold is at the other end of the building but you see a ladder right next to you. What do you do?

Ask someone else to hang the chain fall.
Since the mobile scaffold will fit, go and get scaffold to use. Ladders Last!
It will take longer to get scaffold than to do the job hanging the chain fall. Use the ladder after taking all the needed safety steps.

Proper ladder set-up is critical to ladder safety. How do you do this?

Assess area, ensure a solid stable base, have right ladder for the job, protect ladder from surrounding hazards, secure ladder with tie off, using SLRs with fall protection, get permission on JHA.
Get largest ladder that will fit, tape area off with danger tape.
Stay away from step ladders, these are difficult to secure and hard to fit many work areas.

It seems the only way to reach the work will be to use an extension ladder. You have approval and begin to position the ladder. To fit the ladder to reach the work means the ladder will NOT be at a 4 to 1 ratio. Now what?

The ladder you need has product/material on the sides and rungs. What should you do?

Use the ladder as-is. You have good boots and the ladder shouldn’t be slippery.
Do not use the ladder. A dirty ladder could hide damage or flaws and this could also lead to sa a slip/trip hazard when climbing or descending.
