No need for guard rails if you have a harness and lanyard. Use tie-off.

Tip-over can occur in scaffolds that exceed 4 to 1 in height. Verify the scaffold has been tied into stable surrounding structure.

You need to access a scaffold that has no visible ladder attached. The structure of the scaffold has rungs and cross braces that could easily be climbed. What now?

Get help. All scaffolds must be designed to include safe access stairs/ladder. Most scaffold components are not designed for safe climbing.
If the components will bear your wight, then climbing those will be safe with proper tie-off.
Use the cross braces only for climbing. These allow easy access to all work decks.
