    Carolyn Caseman Evans Memorial Scholarship
  • Scholarship Guidelines  

  • PrimaryPlus is offering FIVE $2,000 post-secondary education scholarship (college, university, technical school, etc.) to a high school senior in the following Kentucky counties: Lewis, Fleming, Mason, Greenup, Carter, Boyd, Bracken, Robertson, Rowan and also including Ripley-Union-Lewis-Huntington Schools in Ohio.  The scholarship program has been named in honor and memory of a great nurse, Carolyn Caseman Evans, who served the patients of PrimaryPlus with great integrity, compassion and a zest for her work.  This program is designed to enhance the education and resulting medical careers of deserving students.  Please read guidelines carefully as some may have changed.   

     

    Participation Requirements: 

    1)      Scholarship candidate must be from one of the counties/schools listed above. 

     2)      Applicant must be a current high school student planning to enroll in an educational or training program at an accredited college, university or technical school.  Students applying for the PrimaryPlus scholarship should be planning to pursue a career in healthcare in some capacity.    

     3)      All applicants must agree to a personal interview with judges and to appear            at a regular Board of Directors meeting of PrimaryPlus—if needed. 

     4)      The high school senior cannot be an immediate family member (living in the household) of the board of directors, employee of PrimaryPlus and/or scholarship selection committee. 

     5)      All applicants must complete a typed, minimum 500, not to exceed 1,000 word essay.  The essay should explain your professional goals and objectives within that of the medical field.  Clearly state in your essay the contributions you expect to make to the community and how this scholarship will help you achieve these goals.  

    6)      All applicates must submit a copy of your high school transcript and or                   ACT/SAT scores.

    7)     All applicants must submit 2 reference letters.  

     

    Limitations on Candidate, Study Discipline and Educational or Training Institution: 

     

    1)      Scholarship will be awarded for one (1) school year only.  The time period generally accepted is August through May, spanning portions of two (2) calendar years. 

     2)      Scholarships will be paid directly to the applicant’s college, university or technical school of choice upon receipt of appropriate evidence that the applicant has been accepted and is enrolled as a full-time student.  No money will be paid directly to the recipient. 

     3)      PrimaryPlus will retain unused scholarship funds.  Unused funds may result if a recipient ceases to be a student. 

     4)      Scholarship will not be renewed under any circumstances.  A single candidate can earn a maximum of one PrimaryPlus scholarship.  Scholarships are not assignable. 

     5)      PrimaryPlus reserves the right to not issue a scholarship if the scholarship committee feels there is not a viable candidate.     

     

    General Selection Criteria: 

    1)      Eligibility determination. 

    2)      Grade Point Average 

             a. Curriculum will be considered as well as grades. 

             b. A copy of applicant’s transcript and ACT or SAT scores must be provided. 

    3)      School and community involvement. 

    4)      Financial need determination. 

    5)      Personal references. 

    6)      Interview with panel judges (if necessary). 

    7)      Essay.

    • Since financial need is one factor in selecting the recipient of the PrimaryPlus Scholarship, the following information is critical to the selection committee in their deliberations. The information is strictly confidential and will only be reviewed by the members of the selection committee.
