  • Submitting our travel questionnaire is simply a means of gathering information about your plans to be able to better answer any questions you may have in regards to travel, and provide you with a timeline and estimate of services. It is not a commitment to services or a guarantee that services are able to be provided in time to meet your travel plans.

  • Wonderful! We may have some of this information in your pet's chart already, but please take the time to fill out the form completely to ensure what we have is up to date.

  • Is your pet traveling domestically or internationally?*
  • Will your pet have a layover in a European country?
  • The European Union requires a separate health certificate issued to your pet, as if the final destination is in Europe. This applies to any duration of layover. This will result in additional health certificate fees, USDA endorsement fees, and return documet shipping fees.

  • This location requires a current Rabies Titer Test

    A rabies titer is a blood test that checks for the number of rabies antibodies your pet has in their body. It must be above a certain level for this location to allow entry.

    Most locations require proof of the two most recent vaccinations, with some locations requiring no lapsed time between the two vaccines. We will need proof of both vacciantions to determine if we need to revaccinate before testing. 

    There are only a couple of labortories in the United States that process this test, which means it can take 3-6 weeks to receive results.

    If your pet(s) already has a current Rabies Titer please attach the official results along with two signed rabies certificates under the pet descriptions later in the form.
