Liability Waiver Form

At YUZU & SPACE, your health and safety are of our utmost importance. Although we might not be able to provide absolute safety amongst our staff, employees, and guests, we assure you that safety precautions and strict compliance to the maintenance of personal hygiene are in place in order to protect everyone's health and safety.

In this regard, please fill out and submit this Liability Waiver Form in order to further proceed with the use of the play space.

Waiver and Release of Liability:

I, the undersigned, am the parent or legal guardian of the child named above. I acknowledge and agree to the following:

1. Assumption of Risk:
- I understand that participation in activities at the indoor play space involves inherent risks, including but not limited to physical injury, property damage, or other unforeseen risks.
- I voluntarily assume all risks associated with my child’s participation in these activities.

2. Release of Liability:
- I, on behalf of myself, my child, and our respective heirs, administrators, and representatives, hereby release and hold harmless YUZU & SPACE, its owners, employees, agents, and affiliates from any and all claims, demands, damages, or causes of action arising from my child’s participation in activities at the indoor play space.

3. Medical Consent:
- In the event of an emergency, I authorize YUZU & SPACE and its representatives to secure medical treatment for my child as deemed necessary. I agree to be responsible for any medical expenses incurred.

4. Rules and Conduct:
- I agree to ensure that my child follows all rules and instructions provided by YUZU & SPACE staff. I understand that failure to adhere to these rules may result in my child being asked to leave the premises without refund.

5. Indemnification:
- I agree to indemnify and hold harmless YUZU & SPACE, its owners, employees, agents, and affiliates from any and all claims, demands, damages, or causes of action arising from any injury or damage caused by my child’s actions during participation in activities at the indoor play space.

By signing below, I acknowledge that I have read, understood, and agreed to the terms of this liability waiver form.
