You are being asked for your permission for your child to receive a diagnostic assessment from Krista Cochrane, Registered Psychologist (APE) #773, a trustee as defined by the Health Information Protection Act (HIPA) and an approved vendor with Jordan's Principle. This comprehensive evaluation will include the administration of standardized tests of intelligence and academic achievement. Other assessment methods may include a clinical interview, a review of school records and previous assessment reports (if applicable), and several observational rating scales.
The exact nature of the assessment will be tailored to meet your child's needs. Depending on your child's stamina and attention span, it may involve one or more testing sessions. Resulting diagnoses may include Intellectual Disability, Attention Deficit Hyperactivity Disorder (ADHD), Learning Disorders, Anxiety, and/or Depression. Screening for other developmental disorders, such as Autism Spectrum Disorder (ASD), may also be included.
Confidentiality
The information obtained for this assessment is considered personal health information andis confidential. In most circumstances, information gained throughout this assessment cannot be released without your written consent. However, there are some situations in which information must be disclosed. Examples include reporting imminent risk of harm to your child or others, suspected neglect or harm to a child, including the witnessing of violence, a court order to release client information, or where required by legislation (e.g., the inability for individuals age 15+ to operate a motor vehicle (Traffic Safety Act), and mandatory reporting of contact with infectious diseases (under the Public Health Act

The fee for this assessment is being covered through Jordan's Principle.
Risks and Benefits of Assessment
Given that this is a private assessment, parents own the report. This implies that they can choose to share it with their child's school or physician at their discretion. Once a diagnosis is provided, parents/guardians can share a copy of the report with various service providers to obtain specialized services. These services may include the implementation of an individualized educational plan at school, funding support (e.g., Jordan's Principle, Canada Saskatchewan grants for postsecondary students, Disability Tax Credit), and/or other specialized services as recommended (e.g., tutoring, speech and language therapy, occupational therapy
Please be aware that information gathered during the assessment, such as observations or test results, may indicate the presence of additional unanticipated difficulties at the time of referral. This may cause some distress. Concerns about this new information will be discussed during the debriefing session.
Parental permission for this assessment is valid for one year from the date of signature. Parents/Legal Guardians have a right to cancel their consent, in writing, at any time. However, no collected records will be destroyed.
