High Performance Venue Use Booking Request Form

Aligned with the Gymnastics WA strategic vision in the 2022-2026 Strategic Plan, Gymnastics WA places a significant empasis on creating avenues for representative opportunities, fostering inspiration and instilling a sense of pride through stellar athletic performances. 

In order to ensure WA high performance athletes receive support on their pathway to podium, Gymnastics WA offer a number of State Gymnastics Centre venue hire options to high performance atheltes, outlined in the High Performance Venue Use Guidelines. 

Athlete's eligible for benefits as part of the High Performance Venue Use Guidelines are unable to book the venue on behalf of themselves. Booking requests must come from a representative of the athlete's affiliated club.

Booking requests are to be submitted via this form by a representative of the athelte's affiliated club. After submission, Gymnastics WA will be in contact for booking confirmation and/or further information.

Note that as per the Facility Use Guidelines (clause 5.1), there is a priority of access which will be followed when managing this booking request (these requests will be classed as 'High Performance programs and events'). 

As part of this booking request, you must agree that you have read and agree to the requirements outlined in the High Performance Venue Use Guidelines, as detailed below. 

• Users of the SGC under the High Performance Venue Use Guidelines must abide by the Facility Usage Guidlines at all times when using the venue, including agreeing to the Terms and Conditions outlined in those guidelines. 
• Athletes must always be accompanied by a suitably qualified coach when using the SGC. 
• Set-up and pack-down of the venue is the responsibility of the user. 
• All athletes using the venue during the booking time must meet the eligibility category the booking has been made under. No athlete who does not meet that category's bounds can use the SGC under the booking made in line with the High Performance Venue Use Guidelines.
