Team Registration for Dodgeball Tournament (ALL PLAYERS MUST BE 18 or OLDER!)

Please complete the form to register your team for the Crown Point Firefighters Association dodgeball tournament benefitting Northwest Indiana Cancer Kids Foundation. Please collect and submit the individual email addresses so we can email the waivers to the players.
REGISTRATIONS ARE DUE SEPTEMBER 27th, 2025!
- EVENT LOCATION: SPARTA DOME 1355 E NORTH STREET, CROWN POINT INDIANA 46307.
- DATE/TIME October 11th, 3PM REGISTRATION, GAMES BEGIN AT 4PM.
