Medication Guidlines
Life Adult Day Academy will administer medication during the day when students are present and medications are needed. These guidelines are very specific by the Waiver and we must follow these at the Academy.
*Medication will not be administered if any of the following guidelines are not met:
*Doctor's orders must be on the file for all medication to be administered, including PRN medications such as Tylenol and cough medicine.
*Medications will only be dispensed from original containers/medication cards. Medications sent in any
other container such as a baggie or plastic cup will not be administered.
*The label on the medication container must match the original order for person, dosage and administration time
*All changes in medication container must match the original order for person, dosage, and administration
time.
*All changes in medication must meet the above requirements.
*If medications are to be given in food or are to be crushed or split these orders must be on the original medical order signed by the physician. LADA does not provide pudding, applesauce, etc for medication administration. These items will need to be provided with the medication.
*It is the responsibility of the residential provider to assure medications and MARS (MedicationAdministration Records) are kept current at the day program. LADA will provide MARS for those students
living at home who will take medications at the Academy.
As guardian/healthcare representative/parent for the above mentioned individual, I authorize Life Adult day academy employees if accompanying this person to exchange medical information with any medical personnel as necessary to meet the current emergency needs.
