Swim Lessons

I understand that all bookings are based on a first come, first served basis.
I understand that when purchasing my swim lesson package, there is a 3% credit card transaction fee for each purchase.
I understand that any cancellation will require 72-hours’ notice to receive a credit for any future lessons that have not been paid for. Without 72 hours’ notice, I will not be subjected to a refund or credit for future lessons.
I understand that there may not be make-up slots available for any cancelled lessons.
I understand that weather cancellations may occur and will result in a credit towards my next lesson, or a make-up date if the instructor’s schedule allows.
I understand that I may not be able to reschedule any lessons due to bookings of other clients.
I understand that due to the nature of a Mobile Swim company, lessons may start 15 minutes early or 15 minutes late due to traffic and the distance of travel from client to client. I will receive my allotted time as scheduled and paid for.
I understand that children 2 years and younger will receive a 20-minute lesson during a 30-minute time slot due to attention span.
I understand that my child is not receiving Infant Self Rescue (ISR) swim lessons, nor is my child’s instructor ISR certified.
I understand that there are certain risks involved with swimming and related activities. I hereby agree to assume all liability for myself and my child during swim lessons.

Release of Liability: I release and hold harmless Island Aquatics NJ, LLC, its owners, operators, pool hosts, instructors and contractors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participants and/or the undersigned, while in or upon the premises under the control and supervision of Island Aquatics NJ, LLC, its owners, operators, pool hosts, or in route to or from any of the said premises. I also acknowledge that enrollment, participation in or completion of lessons in no way implies that the participants are drown proof or water safe and that as the parent or legal guardian of the participants, I am ultimately responsible for my child’s safety in or around the water and must at all times ensure appropriate, vigilant and persistent supervision.*

Assumption of Risk: I understand, agree, and acknowledge that there are risks inherent in sports activities conducted by Island Aquatics NJ, LLC, and its owners, operators, pool hosts, instructors and contractors. These activities may be of hazardous nature and/or may include swimming, diving and a variety of strenuous exercise and physical activities. With the full understanding of the facts, I state that to the best of my knowledge, the participants I am registering have no medical, physical, or emotional conditions that would hinder or prevent safe participation in Island Aquatics NJ, LLC.*

Enrollment Review and Payment Policies: I understand that I am enrolling for swim lessons with Island Aquatics NJ, LLC and that the information submitted on this enrollment form will require review as it is the intent of Island Aquatics, NJ LLC to provide each participant with an individualized, effective and safe learning experience. I acknowledge that in some health-related cases, Island Aquatics, NJ LLC may not be able to provide services. I certify that all medical/health information I have provided is correct and true and agree to provide additional information, including a physician’s clearance if requested, prior to the beginning of any lessons. I understand that in order to secure my child’s selected lesson time I must pay all applicable registration fees per the instructions sent to me. I understand that participant enrollment may be cancelled if payment is not received.**
