1. Player Participation Risk Statement: I, the Parent or Legal Guardian of the above-named participant (the "Player"), agree that the Player will abide by the rules of Raptors Soccer Academy North Broward, LLC. I fully understand that participating in the sport of soccer presents a risk for serious injury and death, and in consideration for Raptors Soccer Academy North Broward, LLC, am accepting Player participation in the soccer program and its activities (the "Programs"). I hereby release, discharge, and/or otherwise indemnify Raptors Soccer Acedmy North Broward, LLC, its affiliated organizations and sponsors, their employees, including the owners of the fields and facilities used for the Programs against any claim by or on behalf of the Player as a result of the Player's participation in the Programs. The Player has received a physical examination by a licensed physician and has been found physically capable of the Programs. I hereby consent to have an athletic trainer and/or doctor of medicine and/or dentist provide the Player with medical assistance and/or treatment in the event of a medical emergency and agree to be responsible financially for the cost of treatment.
2. Sportsmanship: Any negative behavior from player or parents will result in immediate dismissal.

4. Arrival: Sessions start promptly. Be sure to arrive on time.
5. Children not enrolled in the program are required to remain off the field while the session is going on.
6. Attire: Proper attire is required at ALL TIMES. T-shirt, shorts, turf shoes or sneakers and shinguards are mandatory for each session. If your child does not have shinguards, they will not be able to participate.
7. No Smoking At the Park
8. Raptors Soccer Academy North Broward may take photographs or videos of its instructors or students participating in soccer. These images are for the purposes of instruction, promotion or advertisement. It is agreed that any such images become the sole property of the academy and may be used by the academy for any instructional, commercial, or advertising purposes at it’s discretion and any claims for such is waived.
I have read and understand the above academy policies and agree to abide by them and all other posted academy policies.
