I recognise that my trainer is unable to provide me with medical advice and that this information is only used as a guidline to the limitations of my exercise ability. I understand that answering "yes" to any of the above health screen questions will require me to provide a medical clearance before commencing any physical actiyity/exercise.

I have answered all questions to the best of my ability and will not hold snap fitness or my trainer liable for any injuries or conditions that may occur as a result of my training.

Cancelation Policy

Individual 1 on 1 session - LOCK IN 12 WEEKS
·      24 hours notice is required to cancel your scheduled session, the session can then be rebooked at an agreed upon time.
·      Failure to give 24 hours notice or fail to show to your scheduled session will result in the session being forfeited and paid in full.
·      Notice must be given to me directly via phone call or message.

Buddy 2 on 1 session - LOCK IN 12 WEEKS
·      24 hours notice is required to cancel your scheduled session, the session can then be rebooked at an agreed upon time.
·      Failure to give 24 hours notice or fail to show to your scheduled session will result in the session being forfeited and paid in full.
·      Notice must be given to me directly via phone call or message.
·      Late cancelation or failure to show by one client will result in their half of the session being forfeited and paid in full, the remaining client will then pay their half of the scheduled session.
·      Cancelation prior to 24 hours by one client will then make the scheduled session a 1 on 1 session and will be charged at the 1 on 1 rate or can be rescheduled at an agreed upon time.

Online Training - LOCK IN 12 WEEKS

All services are 12 weeks locked in, this ensure adequate time to build new habits and routines and get results. After the 12 weeks services run in 4 week blocks. If you wish to cancel after the first 12 weeks or in the future please give 1 weeks notice otherwise your training and payemtns will continue.
