I would like to cordially invite your company/organization to participate in an exciting community event! Transformation Health, AETNA Better Health, Healing City Baltimore, and DTLR have joined forces to bring healing to the communities of Baltimore and we need your help! This will be our 1st Annual Healing Summer Jam and Grand Opening of The Healing Suites of Transformation Nation. “THE HEALING- Suites” -15, unique Spaces designed to each be an intervention, to promote healing, & resiliency for youth, impacted by trauma. Currently, our programming is for elementary and middle school youth ages 6 to 13.

The Healing SUMMER Jam, a day of healing in Baltimore City! We have requested the Mayor and City Politicians to join us with a Proclamation to seal the commitment. On July 20th from 10am – 3pm, we will launch a day of healing and we would like you to be there! The fair will provide a unique opportunity for health and human service providers like you to market your services, provide demonstrations of your services, and provide give-a-ways to more than five 1200 residents. Since the event is designed to meet the service and information needs of current teens, adults, seniors, and families we are requesting your participation. The Healing Teams ultimate goal is to increase Healthcare awareness; however, we recognize the importance of bringing these services to individuals who would otherwise not access them alone.

Vendor Registration Deadline	Monday, July 15, 5pm but will run thru until vendor spaces are full
COI Deadline	No later than July 12th COB
Cancellation of Attendance	Monday the week of the event by 5pm (PLEASE contact - Cyrus Nusum 443-873-9106)
Set-Up Time	Day of 8am - 9am no exceptions!  You are more than welcome to arrive as early as 7am.

1. Your table space will be provided with two chairs. There will not be any additional chairs so please bring any additional chairs you may need.

2. Table location will be pre-set based on your agency’s services. There is No Guarantee of particular table location.

3. Early Set-Up: Early set up will be as early as 7am. Please contact Cyrus Nusum for coordination. Transformation Nation, Inc. and their affiliates are not responsible for any items left unattended.

4. Street parking is available upon availability. Get there early. Please do not block other vehicles and remove your vehicle as quickly as possible after unloading to avoid ticketing. 5. Unregistered Vendors/solicitors are not allowed on the premises due to space. The Event Planning Committee staff/volunteers will ask the solicitors to leave.

6. Any Vender who has his own tent is more than welcome to bring it to set up in your designated hub.

All COI's must include that MLK OFFICE EQUITIES, LLC, RA RETAIL EQUITIES, LLC & WALLACE H. CAMPBELL & CO., INC., & TRANSFORMATION NATION, INC. & TRANSFORMATION HEALTH, LLC are listed as “additional insured” for General Liability. If you are not familiar with how to request a Certificate of Insurance from your insurance company, contact your insurance company directly to request your COI. This COI is only required for one day. THE INSURANCE RIDER DOES NOT NEED TO GO BEYOND JULY 20TH. One day riders can be as low as $100 for the day depending on your insurance company.

The Healing Summer Jam will begin with a peace walk starting at the Transformation Nation,Inc. parking lot and will go west bound up Saratoga St. to Schroeder, and past POE Homes Housing Projects and then backdown to the TNation parking lot. Once returning to the site the event will kick off for the day. There will be resource hubs around the parking lot with specific community recourse HUBS for attendees to visit. Each attendee will receive a health passport to get a stamp from at least one vendor in each resource hub. Once their passport is complete, they will be entered to WIN a special gift.
