SOLO TITLE STUDENT AGREEMENT

I,   Contestant First Name*   Contestant Last Name*   , residing at   Street Address*      City*   State*   Zip* ,
 if I win the Dance Masters of America, Inc. Title of    Please Select Petite Miss Master Junior Miss Junior Mister Teen Miss Teen Mister Miss Mister *   Dance of America (“Title”), a Title protected under Federal Copyright Laws, do hereby acknowledge and agree to the following:
1. That, during the time of my reign, which shall be defined as the span of time between the date of this Agreement is signed and executed and the date my Title is relinquished, I will represent Dance Masters of America, Inc. to the best of my ability.
2. That, Dance Masters of America’s, Miss or Mr. title winners may hold other dance related titles prior to entering this competition. Upon winning Dance Masters of America’s Miss or Mr. title, they will be required to relinquish any currently held national titles before signing this agreement.
3. That, during the time of my reign, I will not compete for any other national dance title for a 60-day period after signing this agreement.
4. That, during the time of my reign, I will follow the Dance Masters of America, Inc. Solo Title Scholarship Competition Rules, which I acknowledge have been provided to me.
5. That, during the time of my reign, I am a representative of Dance Master of America, Inc., and, as such, my behavior must be exemplary, and my attire and overall appearance will be appropriate and fitting of such representation. 
6. That, during the time of my reign, I will be a role model for other dancers and will be of good moral character, and I will refrain from any behavior that may tarnish, damage or otherwise negatively affect the reputation and goodwill, or may be objectionable to Dance Masters of America, Inc. This includes, but is not limited to, use of foul language, inappropriate posts to social media, charges or convictions of any offense under federal, state or local laws, or any word, act or deed not in accordance with public morals, and standards.
7. That, when attending any event or convention held by Dance Masters of America, Inc. any/or any of its affiliated chapters, I will be an active and willing participant, helpful, cooperative, compassionate, courteous, friendly, and a role mode to those in attendance at all times. I will arrive on the date and time specified, and I will assist in any way I am asked. I will attend all meetings, rehearsals and performances on time, well-groomed and dressed appropriately. 
8. That, during the time of my reign, I will notify Dance Masters of America, Inc., prior to my acceptance to participate in any performance or event, other than my studio events. 
9. That, I may never again compete for the Title I now hold.
10. That, upon submission of a scholarship monetary award voucher to the National Treasurer before August 31st of the year in which this Title was awarded, my Teacher of Record will receive fifty percent (50%) of the scholarship monies awarded. The remaining fifty percent (50%) will be distributed to my teacher of record, upon submission of scholarship award voucher to the National Treasurer before August 31st of the year in which I relinquish this Title, after full compliance with this Agreement.
11. That, during the time of my reign, I will continue to study with my Teacher Of Record, who submitted my application for this Title, on a weekly basis, provided that my Teacher of Record continues to offer classes and remains a member in good standing, until such time that I relinquish this Title.
12. That, in the event of any violation of this Agreement, Dance Masters of America, Inc. shall have the right, in its sole discretion, to take any action it deems appropriate, including but not limited to rescinding the benefits of my Title, along with any amenities, awards, scholarships and/or privileges awarded or associated with my Title.
13. That, if my title is rescinded, I will cease the using my Title in conjunction with my name, photograph or likeness shall cease.
14. That, during the time of my reign, my name and likeness may not be used without my National Title in any dance related publication.
15. That, if I am represented by an agent/manager, (s)he must be notified of this agreement and comply with the requirements. 
16. That, in the event I am unable or unwilling to fulfill the duties of said title or cannot return to the next annual convention, I will immediately notify my Teacher of Record and Dance Masters of America, Inc. in writing and forfeit to my successor, the benefits of title, all remaining rights to amenities, awards, scholarships, and privileges.
17. That, this Agreement shall be construed in accordance with the laws of the State of Maryland.  

MEDICAL AUTHORIZATION

I,   Parent First Name*   Parent Last Name*   , am the parent and legal guardian of         Contestant First Name*   Contestant Last Name*   a minor child (under the age of eighteen (18) year of age), give my permission to Dance Masters of America, Inc., the Officers of the Organization, Directors, and Assistants of the Competition to obtain whatever medical and/or hospital care and treatment may be deemed necessary at Dance Masters of America, Inc. National Convention, and the Anaheim Marriott, from June 27th through July 6th, 2024.
