I, the undersigned, on behalf of myself or as the parent or legal guardian of the child(ren) listed on this application, give permission for my child(ren) to participate in the Piedmont Athletic Club tennis and pickleball programs. I hereby assume full responsibility for all risk of injury or loss which may result from participation in this activity, and hereby agree to hold harmless, release and forever discharge the instructors, Piedmont Athletic Club, its agents and employees from any and all claims and demands whatsoever which the undersigned or any third party may have against any accident, illness, injury or death or damage to or loss of or destruction of property arising or resulting directly or indirectly from participation in these programs. I understand, agree and acknowledge that some activities may be of a hazardous nature and/or include physical and or/strenuous activity. Understanding this, I state to the best of my knowledge, that the participant(s) does not have any physical, mental or emotional health conditions that would hinder active participation in the programs.
I grant Piedmont Athletic Club, its representatives and employees, the right to use mine or my child(ren)’s photograph for purposes of publicity, illustration, advertising and web content.
Program sessions are non-refundable, non-transferrable to another session and cannot be made up.
By signing this agreement, I agree to all the above-mentioned terms and conditions and agree to make payment for the amount due to Tuyet Cahill to the first class.
