PERSONAL INJURY RELEASE AND WAIVER OF LIABILITY: I hereby acknowledge and understand that Martial Arts are physical activities and learning self-defense skills involves some physical contact. Renzo Gracie Pearland (Hanuman BJJ Group LLC.) or its staff and students are not to be held liable or responsible for any risks and/or injuries as a result of training Martial Arts. The above person consents to the student participating in the classes/training and are aware of the risks involved in such use. The people-stated in this contract hereby hold Renzo Gracie Pearland, it’s successors, assigns, owners, officers, directors, employees, and agents harmless from all claims which may be brought against them by the people stated in this contract, student or on student’s behalf for any such injuries or claims and do hereby forever release and discharge Renzo Gracie Pearland, its successors, assigns, owners, officers, directors, employees, and agents from all claims, demands, injuries, damages, actions, losses and expenses. *PARENT/GUARDIAN CONSENT: The above said person is the legal parent or guardian of the student participating in this introductory. The above person consents to the student participating in the classes/ training and is aware of the risks involved in such participation. AUTHORIZATION AND RELEASE FOR PROMOTIONAL VIDEOS AND PHOTOS: Hanuman BJJ Group LLC appreciates your positive comments about our business. From time to time, we will promote Hanuman BJJ Group LLC, with videos, photos and other forms of social media, which may include member comments, photos, and names. The purpose of this Authorization and Release is to notify you that we may want to use your comments, photo and name in our marketing videos, photos, brochures and prospect folders. By your signature below, you: (i) authorize us to use your comments, photo and name in any photo, video or social media, as the case may be, for marketing purposes without providing you with any compensation or consideration; and (ii) release Hanuman BJJ Group LLC from any and all claims relating to the use of your comments, photo or name on any photos, videos or social media in connection with marketing Hanuman BJJ Group LLC. The undersigned hereby agrees to the terms of this Authorization and Release as set forth above.
