Communities That Care Application

Tell us about your community and why you think you should be awarded. Please include information on local resources (or the lack thereof), special populations within your community, and any local data and information if possible.

• If applicable, please include the rural nature of your community and identify the high-risk health factors you believe are affecting youth in your area with data from the 2023 South Dakota State Health Assessment.

Please disclose if there is already a functioning prevention coalition in your community.

• Complete the Budget template, focusing on salary for 1 FTE facilitator, meeting expenses, community board organization, youth risk survey execution, and promotion of the coalition (and attach as an Excel workbook with your application).
• Successful applicants will work with the South Dakota Department of Health to develop a final budget but will submit a budget for up to $60,000 for Year 1 per community with this application. Final funding amounts for each community will be determined by SDDOH based on need and current capacity level. This funding will include a line item for one full-time coordinator/facilitator, a youth survey, marketing, travel, supplies, and ancillary expenses.  
• Workplan: Upon review of the links below, create a brief timeline of how you would work through the 5 Phases of CTC and what you will aim to accomplish each month of the 2-year project period. The workplan should encompass the CTC Milestones.

• Selected communities will work with SDDOH & SD CTC Coach to complete a final 2-year plan upon award.

Applicants should attach letters of support from 2 partner organizations, and 1 from the fiscal agent if different from the applicant organization. Letters should be written by individuals who are authorized to speak on behalf of the organization.
