I agree to comply with all instructions given by my tattoo artist regarding the after care.
I understand and acknowledge that:
The process of tattooing involves piercing the skin with a tiny needle and depositing ink into the skin.this process may cause pain, swelling , discomfort & bleeding.

There is a possibility of an allergic reaction to the ink, and I have informed my tattoo artist of any allergies or medical conditions that may affect the tattoo process.

The tattoo artist has explained the proper aftercare.

The tattoo artist has disclosed that the tattoo can not be removed, and that any attempt doing so can cause scarring, skin discoloration etc.

The tattoo artist has disclosed that the final result of the tattoo may vary from the initial design due to factors such as skin texture, aftercare healing.

I herby release the tattoo artist from any and all liability, including but not limited to:
Injury, infection, allergic reaction.

I understand that the tattoo artist is not responsible for any medical expenses or other costs incurred as a result of complications arising from the tattoo process.
