I acknowledge that I have discussed the procedure and any potential risks with my technician at Lucky Links.
I understand that although rare, an allergic reaction to the jewelry may occur.
I understand that the welding process involves a small spark and carries minor risks, including but not limited to irritation, discomfort, burns, or scarring.
I understand not to look directly at the welding spark during the process, as this can affect my eyesight.
I agree to follow the jewelry care instructions provided by my technician.
I understand that while permanent jewelry is designed to be long-lasting, durability can vary depending on lifestyle, daily activities, and care.
I understand that there may be times when I need to remove my jewelry, including for certain medical procedures such as MRIs or surgeries. I can do so by cutting at the jump ring and may return to Lucky Links to have it re-welded.
I understand that my technician will check the weld before completing the service.
I understand that Lucky Links stands behind the quality of its work. If my jewelry breaks within 14 days of my appointment, it will be replaced at no cost.
I understand that Lucky Links offers complimentary rewelding and resizing for jewelry originally purchased from Lucky Links if it breaks at the weld. Breaks outside of the weld point are not guaranteed to be repairable.
I understand that rewelding jewelry not originally purchased from Lucky Links is available for a $15 fee per weld. I acknowledge that repairs on outside jewelry are done at my own risk, and Lucky Links cannot guarantee the quality or durability of materials not sourced directly from Lucky Links.
I understand that all sales are final once the chain has been cut to size.
I confirm that I have received aftercare instructions and understand how to properly care for my jewelry.
Consent & Release:
I voluntarily consent to the permanent jewelry service and understand the nature of the procedure. I release Lucky Links, its technicians, and affiliates from liability for any complications or injuries that may occur.
Age Verification:
I confirm that I am at least 18 years of age, or I have consent from a parent or legal guardian.
Photo Release:
I give permission for photos or videos of my jewelry and/or the process to be used for documentation and marketing purposes.
Signature:
I have read and understood this consent form and agree to proceed with the service.
