USBC Membership Card Request Form

Request must be received by 20th of the month.
Membership Card policy:
- USBC Membership Cards are for currently active (unexpired) members
- USBC Membership Cards expire when your membership expires.
- Membership cards will be created in one monthly batch at the end of the month
- Membership cards will be available for pick-up in person at the following General Assembly or Social Saturday or it can be requested to be dropped off at the member's mailbox at one of the local Rosarito mail rooms.
- Membership Card expiration date is listed on the back of the card. A new card will be issued upon renewal of your membership, if requested.
- There is a $25 USD fee to replace lost or damaged Membership Cards
