This form should only be used for student activities related transactions.
You will use this to initiate the creation of a contract and payment to a vender. Once we receive this, we will send the contract and A19, along with a request for a W9 to the guest artist and notify you once it has been signed and executed.

Please be specific of any other guest requests (ie request water bottles on stage for presentation), technical requirements (ie need to have a lapel mic or access a piano), and/or special conditions (these are typically only used for high cost performers and you will work with Richard Arquette to confirm those requirements are accessible to the campus).
