• Learn to Turn Youth Program Registration Form

    Thank you for registering, mentoring and supporting a youth woodturner at the AAW Symposium. Please complete this form to register a youth participant. Note: we ask that all youth mentors who register a youth participant volunteer in the Youth Room. For each youth accompanied to the Symposium by you, please plan one volunteer shift. As a volunteer, you will play a key role in helping the instructor during the rotation by observing, coaching, and offering guidance to ensure the delivery of high-quality, safe, and effective hands-on training for the participating youth. Your involvement contributes to creating a positive and enriching learning experience for the next generation of woodturners.

  • AGREEMENT

    In exchange for participation in the youth activity of hands-on woodturning classes organized by American Association of Woodturners (AAW), the undersigned, parent or legal guardian of the participants, hereby agrees to the following terms:

     

    Terms

    1. Participant(s) agree to observe and obey all posted rules and warnings, and further agrees to follow any written or oral instructions or directions given by the youth class instructors and volunteers.
    2. The parent or legal guardian of participant(s) recognizes that there are inherent risks associated with the activity of woodturning and assumes full responsibility for any personal injury to Participant(s), and further release and discharges AAW and the Oregon Convention Center for any injury, loss or damage arising out of Participant’s participation and use of, or presence upon, the facilities of the Oregon Convention Center  whether resulting from the fault of Participant(s) or any other third parties. The parent or legal guardian and Participant(s) agree(s) to indemnify, hold harmless, and defend AAW and the Oregon Convention Center against all claims, causes of action, damages, judgments, costs or expenses, including attorney’s fees and other litigation costs, which may in any way arise from Participant(s) involvement and use of, or presence upon, the facilities of the Oregon Convention Center.
    3. This Agreement and each of its terms are the product of an arms’ length negotiation between the parties. In the event any ambiguity is found to exist in the interpretation of the agreement, or any of its provisions, the parties, and each of them, explicitly reject the application of any legal or equitable rule of interpretation which would lead to a construction either “for” or “against” a particular party based upon their status as the drafter of a specific term, language, or provision giving rise to such ambiguity.

    Note: If an on-site telephone number is not provided, the parent or guardian must either be present during the woodturning instruction or provide an authorization for medical treatment to the registrar located at the entrance to the youth room and who will be checking in each of the participants.

     

    I, the undersigned parent or legal guardian, have read this document, and am signing it freely.

    I understand the legal consequences of signing this document, including (a) releasing AAW from all liability on Participant(s) behalf, (b) waiving the right to sue AAW, (c) and assuming all risks of Participant(s) participation in this activity, including travel to and from the activity or any events incidental to this activity.

    I understand that I am responsible for the obligations and acts of the Participant(s) as described in this document. I agree to be bound by the terms of this agreement.

    If a parent or guardian is not able to be present an authorization for medical treatment will be provided.

  • Parental Release Waiver for Photography

  • I understand that AAW may photograph participants of the youth demonstrations for use on the AAW website and in publications, including the American Woodturner journal, and in publicity for the symposium. AAW photography may also include video of youth participants, as well as of their turnings, which are displayed at the Symposium.

    Photography by non-AAW individuals may also take place during the youth demonstration activities. The AAW bears no responsibility and cannot police photographic activities of non-AAW personnel.
