2025 SERVE, INC. ADOPT-A-FAMILY APPLICATION

APPLICATION MUST BE COMPLETED IN FULL TO BE CONSIDERED
Those who wish to receive Christmas gifts and/or food for themselves, their family, or their children, must read the following before continuing:

1. CHRISTMAS GIFTS FOR YOUR CHILDREN:
- If you wish to apply for gifts for your dependent children between the ages of newborn and 18, your child must be residing with you in Callaway County, and you must provide documentation.
- If your child is 18, they must still be high school.
- You will be required to show AND ATTACH TO THIS APPLICATION the following documentation:
  - Your Photo ID
  - Proof of income for all members of the household
  - One of the following for children: social security card, birth certificate, divorce decree with documentation of custody, guardianship papers
  - Proof of residency
- Gifts must be picked up on Thursday, December 11th.
- If your family is approved, the eligible children will receive four Christmas gifts each (something they want, something they need, something to wear, something to read).
- If you qualify, you will also receive a meal basket (groceries) for a full Christmas dinner for your household (a ham or turkey, fruit, side dishes, and a dessert).
2. CHRISTMAS GIFT BAG FOR SENIORS (OVER 60) AND DISABLED ADULTS (OVER 18):
- Any member of the household who is over 60 years old and/or is a disabled adult over the age of 18 may qualify to receive a special Christmas Gift Bag. These are gift bags that include a variety of small stocking stuffers such as socks, work books, hygiene products, etc.. These items will vary year to year.
- Each adult wishing to receive this gift bag must complete a separate application.
- If you qualify, you will also receive a meal basket (groceries) for a full Christmas dinner for your household (a ham or turkey, fruit, side dishes, and a dessert). You may only get one food basket per household. *If there are four Seniors or Disabled Adults in your household, you will be receiving one meal basket for the entire household, not four separate meal baskets.

- Each child is paired with an adopter. If you have multiple children, it’s possible your family may have multiple adopters shopping for the children, which means they may receive different levels of gifts. While we recommend each adopter spend $50 per child, and purchase four items (something they want, something they need, something to wear and something to read) we cannot guarantee that.
