IMPORTANT: Please fill-out one form entry FOR EACH AND EVERY person who will attend the event and who might bowl. We hope all can bowl (since it is a 'bowling party'! 😉) Everyone of all ages is invited to bowl, if they can. Your final "per person" price will be determined by how many games you choose to bowl and whether or not you will rent bowling shoes from the bowling center. Remember, we need to know who will attend and bowl by end-of-day Monday, March 4th. Thanks!

What is this bowler's "bowling name"?
This is your "game-name" that will appear on the scoreboard and could be anything you like. We will have helpers setting up the scoreboards, so it will help if you can provide this information in advance.
How many games will this bowler play?
Please answer this question carefully since it will be difficult to change the day of the event. 3 games is considered average and allows you to warm-up as well as get a good workout. Some kids may only want to do 2, while some more avid bowlers may do 4.
This bowler's shoe size?
Please enter briefly the number with perhaps your gender. Every bowler MUST wear bowling shoes. However, if you will be bringing your own bowling shoes, you can enter "None needed".
