    Volunteer Application Form
    JK Foundation
  • We are so pleased that you have decided to get involved with the JK Foundation in empowering, supporting and mentoring regional young people. Thank you! We really look forward to having you on the team and to making a difference in the lives of young people together.

We appreciate you taking the time to sign up as a volunteer with us and this involves a few steps:

    • The completion of Online ChildSafe training. Please contact Kent Morgan (see below) to get a link and complete this training through the JKF account.
    • Complete this form and provide two referees we can speak with about your suitability to volunteer with children and young people.
    • You will also sign a declaration regarding your suitability to work with children and to abide by policies and procedures as part of this form.
    • Have a valid WA Working with Children Check.

Your volunteer application will then be processed and approved.

We value everyone and take the safety and well-being of the young people we engage with really seriously. Along with good program safety planning, the screening and appointment of volunteers is a vital part of ensuring everyone is safe. You must have a WA Working with Children Check card and will be asked to upload it and you will be asked for the names of referees who can vouch for you. You will also be asked to complete an online training module and to upload the completion certificate.

If you have any issues or concerns in completing this process or in becoming a volunteer with the JK Foundation, please contact Kent Morgan: kentm@jkfoundation.com.au or 0417 952 057.

Thank you and we look forward to working with you!

In the event of an emergency, if my emergency contact cannot be reached, I authorise the staff and leaders of the JKF to obtain medical advice and care that they deem necessary. I further authorise qualified medical practitioners to administer required care, including anesthetic or blood transfusion.
