    Necessary Documentation: To ensure the smooth issuance of approvals, please ensure you have the following essentials at hand:

    1. Your Health Card 
    2. A Valid Identification Document 
    3. Doctor's Request for Diagnostics 

    Hassle-free Uploads: Capture images of your ID, health card, and doctor's request for seamless submission.

    Swift Processing: Recognizing the importance of time, requests received by 4:00 pm will be promptly processed on the same day. For submissions beyond this time, rest assured they will be prioritized first thing on the following day!
     

    Immediate Confirmation: Upon successful reception of your request, expect a confirmation email in your inbox. Furthermore, upon approval of your LOA, another notification shall be dispatched to you forthwith!

    Letter of Authorization (LOA) Validity: 

    For Consultations: Validity extends to the day of approval.
    For Diagnostic Services: Valid for a period of 3 days from the approval date.

    Reapproval Facility: Should your LOA remain unused, simply submit a reapproval request online prior to your subsequent visit!

    Cancellation of Transaction: If you are not able to do your approved procedure transaction based on the validity period, please contact our HMO Concierge to cancel the transaction.

    • Consultation approvals are valid for 3 days only.
    • Diagnostic/procedure approvals are valid for 3 days.
    • Once transactions are approved and processed with an invoice/charge slip, charges will be automatically applied to your HMO Card.
    • If you need to reschedule your approved transaction before the scheduled date, please contact our HMO Concierge team at 09178630992.
  • PROCESS: 

    STEP 1 : Print 1 copy of  your confirmation charge slip 


    STEP 2 : Proceed to the Servicing Department (Laboratory, Imaging, Cardio/Pulmonary) 


    STEP 3 : Present your Printed Chargeslip/Invoice to the Department Clerk for procedure.
