Pet Adoption Contract and Waiver

On the Date listed above this contract is made by and between:

The dog will be spayed and microchipped on or before the date indicated below; the appointment will be made by BF&H: 3/12/25

Spay/Neuter/Microchip/Breeding Agreement:

If above named pet being adopted has NOT yet been spayed or neutered, the adoptive owner agrees to have this done by eight months of age if applicable and within 30 days if the anumal is an adult.

Any donation DOES NOT INCLUDE spay/neuter, pre-anesthesia blood testing, which if elected, is to be paid for by the adoptive owner, and NO REGULAR EXAMINATIONS of this dog or cat are covered by the adoption fee nor will they be paid for by BF&H unless otherwise stated and authorized below. 

In addition, I, the adoptive owner named in this contract, agree that I will have the above named pet spayed or neutered as soon as possible at a vet clinic and I agree to send proof to BF&H via email at bestfriendsandhens@gmx.com if the animal has not been altered before adoption. The adopter also agrees if the surgery is done at a vet of their choice the microchip will be co-registered to Best Friends and Hens, Inc. and the adoptive owner if possible, but the default registration will be to BF&H. The appointment for the spay/neuter/microchip will be scheduled by BF&H unless otherwise agreed to.

All other procedures, treatments, examinations and/or vaccinations, other than specifically noted on this contract, are the financial responsibility of the adoptive owner.

As the animal is not to be bred any litters resulting from an unaltered animal become the property of BF&H & a $5,000 fee will be assessed for the unauthorized breeding.

NOTE: If pet is not spayed/neutered and microchipped by the date designated, BF&H has the right to reclaim the animal and terminate this Adoption Contract.

NOTE to Veterinarian - The following treatments, procedures and/or vaccinations, if any, are to be billed to the BF&H ONLY if agreed upon and stated in writing below and ONLY if an approved BF&H representative schedules and arranges the appointment in advance on behalf of the adopter.

Best Friends and Hens Pet Adoption Contract

As adoptive owner, I agree to the following terms and conditions: (Please Read Carefully)

I understand that it is in the best interest of myself and my other pets (if any) to take the pet, named and described on page (1) of this contract, to a licensed veterinarian within three days from the date of adoption for a routine physical examination and provide proof to the BF&H. The cost of this examination is the responsibility of the adoptive owner and will not be billed to or paid for by BF&H.

1. I agree to keep the adopted pet in a healthy state by maintaining regularly scheduled visits to a licensed veterinarian for vaccinations, including rabies vaccinations according to GA law, a heartworm & flea/tick preventative, and other necessary medical needs as determined by the veterinarian. I also agree to provide documentation of vaccines and preventative to BF&H upon request, and to give BF&H permission to contact my vet to confirm vaccine history, flea/heartworm prevention & tests, and all medical treatments.
2. I agree, if not already altered, to have this pet spayed/neutered and microchipped as defined in the "Spay/Neuter Agreement" section of this contract. I further agree if this isn't done within the time frame above a representative of BF&H has the right to reclaim the pet for the organization.
3. I agree to care for this pet in a humane manner by providing it with adequate food, water, shelter, and behavioral training if necessary. This pet is now, after all, a member of my family.
4. I agree to be a responsible guardian by providing adequate daily exercise and properly containing my pet to my property. 
5. I agree to keep a proper identification tag on my pet with my current phone number to be worn at all times. 
6. You understand and agree your adopted pet will be microchipped before placement (if age appropriate) & registered to BF&H & owner when possible, otherwise if not done before adoption it will be done during the spay/neuter surgery. In an effort to thoroughly explain our microchip policy and retain transparency please understand the microchip is registered according to the microchip registry guidelines, whenever possible we will co-register the microchip.
7. I agree that if my pet is lost, I will immediately notify BF&H by calling 423-903-6518 or emailing info to bestfriendsandhens@gmx.com.
8. You understand and agree once the adoption is final you are now the owner of the pet, however; the microchip registration will never be solely in the adoptive owner’s name, it stays in both names (when possible), this is for the safety and protection of the pet over their lifetime.  
9. I agree I will notify BF&H within ten (10) days of a change of address so they can update the adoption record, this is so your pet can be reunited with you in the event it is lost and we are contacted.
10. I understand and agree this pet is being adopted for myself, will be kept only as a domesticated house pet, and will not be sold, bred, adopted to or given to another person or organization for any reason.
11. I understand and agree, if I cannot keep the pet, I must contact Lindsey Gorman, phone 423-903-6518 or email bestfriendsandhens@gmx.com. Upon contact to BF&H I must advise of my inability to keep said pet and only upon written authorization from Lindsey Gorman am I allowed to re-home said pet. If authorization to re-home is granted by BF&H I hereby agree to release Lindsey Gorman, Board Members & BF&H from liability for any injury or damages to people or property caused by the said pet, and from any causes of action, claims, suits, or demands that may arise from such injury or damages. If authorization isn't granted I must assist with the pet's return or placement to BF&H by holding the pet in a  safe environment until alternative arrangements can be made, and I will provide necessary transportation and all veterinary records to BF&H.
12. I agree to register the pet in behavioral training (if necessary) and will socialize the pet with other people & pets to ensure the pet is well adjusted to all situations. 
13. I understand and agree to allow a representative of BF&H to do a wellness check at any time to check on the welfare of the pet.
14. I agree that if BF&H does a wellness check and BF&H determines the home environment is no longer satisfactory per the organizations guidelines or no longer safe for the pet that BF&H will reclaim the pet with no refund of any donations.
15. By signing this contract, I am agreeing that no one in the home has ever been investigated or charged with domestic violence, animal neglect or animal abuse and if such findings are discovered at a later date BF&H has the right to reclaim the pet with no refund of any donations. 
16. I understand that if I fail to abide by any one of the terms and conditions agreed upon in this contract, Lindsey Gorman reserves the right to reclaim the pet and terminate adoption with NO REFUND of any donations.
17. Should legal action become necessary to enforce the terms of this adoption contract the adopter will pay all legal fees for Lindsey Gorman & BF&H if BF&H is found to be the prevailing party in the suit.

Any and all actions to enforce this adoption contract shall take place in Walker County, GA and Georgia law shall apply as to the enforcement and outcome of this contract.

By signing below the Adopter agrees to all the terms and conditions within this contract.

BF&H agrees to provide Adoptive Owner with an electronic copy of this Pet Adoption Contract and medical information available for said pet.

Placement is to be on the date: 3/31/2025
