  • I have been offered and advised of travel protection / trip insurance options and fees by my travel agent, Janice Neves.

  • I choose to waive travel insurance as quoted by my agent.

  • By declining travel insurance, I understand and agree that I and all travelers on the same booking: 

    • Waive all insurance protection and any assistance from Agency relating to travel changes, interruptions, cancellations, baggage delays or loss, travel supplier defaults, medical costs, accidents, emergencies, injuries, casualties, and any other loss, cost, or event that may occur during the course of travel;

    • May lose up to 100% of the cost of travel bookings, plus any cancellation or related fees charged by Agency and third-party travel suppliers;

    • Fully accept all personal risks, losses, and costs relating to travel bookings; and

    • Release Agency, its directors, employees, and contractors from responsibility for any consequences, known and unknown, resulting from refusal of travel insurance.

  • I decline the offer to purchase travel protection / trip insurance. I understand that I am solely responsible for any cancellation penalties and out-of-pocket expenses incurred. I will also make my own separate travel, medical and any other provisions in the event of an emergency while I am traveling. I also understand that I am not protected from loss in the event of any travel vendor, travel supplier or any travel-related operator default. This waiver confirms that I voluntarily decline travel insurance and travel protection insurance for the trip described above. I understand I am solely liable for all airline fees, supplier fees, and agency fees that may apply, and I hereby release Janice Neves, Seven Sea Journeys and Cruises and Tours Unlimited from any and all liability related to the trip described above.
