IF YOU DO NOT UNDERSTAND ANY PART OF THE DOCUMENT, WE ENCOURAGE YOU TO REVIEW IT WITH YOUR ATTORNEY BEFORE SIGNING. THE PARTICIPANT(S) FOR HIMSELF/HERSELF AND ON BEHALF OF HIS/HER HEIRS, ASSIGNS, PERSONAL REPRESENTATIVES, AND NEXT OF KIN, RELEASE AND DISCHARGE MINIATURE SCHNAUZER RESCUE OF HOUSTON, INC. (“MSRH” OR THE “RESCUE”) AS FOLLOWS:

1.      Disclosure of risk: I am fully aware that fostering any animal through MSRH or volunteering for the Rescue has known and unknown risks. I acknowledge that fostering any animal or volunteering at any event that involves contact with rescue animals carries potential risk of injury, loss of property, and physical harm. Risks include but are not limited to: animal bites, physical injury from lifting or moving animals, traffic accidents occasioned during transport, trips, slips and falls, exposure to diseases and parasites, and general hazards of interacting with the public.

2.      Insurance and physical condition: I understand that my involvement with MSRH is completely voluntary, and that I may cease my involvement with MSRH at my sole election at any time, subject to returning to MSRH any animals in my care at such time. I understand that I am solely responsible for any injury, loss, or damage that I may incur as a result of my involvement with MSRH. To the best of my knowledge, I am in good health and have no physical condition, disability, or injury that would prevent me from my participation in fostering or volunteering with MSRH or present a hazard to any animals in my care.

3.      I agree that by my signature below, and by my participation as a volunteer, participant or foster with MSRH, I freely and voluntarily assume all risks inherent in my involvement with MSRH. Further, I release MSRH, its founders, directors, officers, volunteers, agents, or representatives, from all liability for any loss, damage, injury, or expense that the participant (or his/her next of kin) may suffer, arising out of my participation in MSRH.

BY MY SIGNATURE BELOW, CERTIFY THAT I HAVE READ AND UNDERSTOOD THE ABOVE WAIVER AND RELEASE, AND FULLY UNDERSTAND THAT THIS DOCUMENT IS A RELEASE OF LIABILITY AND I MAY BE GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT.

 

I understand and agree that I am signing this document electronically.

I hereby consent to the legally binding terms and conditions of this document. I further agree that my signature on this document is as valid as if I signed the document in writing. By signing this document, I also represent that I am legally qualified and authorized to enter into this Agreement.

I may decline to electronically sign this document and may subsequently withdraw my consent to sign this document electronically by contacting MSRH. I may also contact the requester to sign this document on paper or to receive a paper copy of the signed document.
