350-word essay typed in 12-point Times Union Font (double-spaced) effectively responding to the following questions:
 
  i. What are your specific educational plans and career goals and why?
  ii. What inspires you to achieve them?
  iii. How have you helped your family or made your community a better place to live?
 
Two Letters of Recommendation on proper letterhead. (one must be from a guidance counselor at your school)
