Read each section before responding
It's a good idea to read through all questions in a section before responding.
To save your application
Scroll down to the bottom of the contact section or the bottom of the form and click on the Save button. Jotform will email you the link to continue editing your form.
De-identified application
As your application is de-identified before review, the selection panel will see your auto-generated application ID along with sections 3 - 7.
Use clear, plain language
Your application will be assessed on clarity and detail. As the selection panel includes a non-surgeon, please explain your activity clearly and use plain language as much as possible.
Inputting your text
To safe-guard losing any work in an online form, it's a good idea to draft your responses (eg in a Word doc) and paste text - please ensure you paste 'clean' (unformatted) text. The following key combinations will work in most browsers to paste 'clean' text:
PC: control, shift, V
Mac: command, shift, V
Long-form response boxes can be expanded to full screen to more easily input and check your response. Within the response box, click on the top right corner 'x' symbol.
Word count
All response boxes contain a maximum word count indicated by the number below the box. For example: 0/150 = the number of words that counts down to the 150 word limit as a response is entered. As a guide, a suggested minimum word count is included on some responses to indicate the preferred level of detail eg: (word guide minimum 100)
