WCAS 2024-25 Team Tryout Registration Form

Tryout Dates: June 9-15, 2024

Thank you for your interest in joining one of our World Class All Star Dance Teams! We look forward to seeing your dancer grow through this exciting process.

All Tryout Week activities will be at WCAS - 717 Crown Industrial Court, Suite K, Chesterfield MO 63005. If you are unable to participate in any part of tryout week due to major schedule conflicts (i.e. already planned vacations or unavoidable major family events such as weddings), please email mistie@worldclassallstar.dance with your date conflict(s) so that we may explore possible options.

Please complete this form and pay the $350 non-refundable Tryout Registration fee to confirm your dancer's tryout registration. This fee covers:

One (1) admission to one (1) of our May or June Tryout Prep Clinics (dates/times TBA).
All tryout week classes and open practice times
Written tryout evaluation (emailed to each participant by June 24th.)

Tryout fees are NON-REFUNDABLE.

Detailed information about our program, schedules for tryout week, and required forms are available via our tryout Band group. Text 314-858-6464 or email mistie@worldclassallstar.dance to join the Band group, which will be updated with additional information as we get closer to our June tryouts.

Dancers will not be allowed to participate in tryout week without a valid registration, paid tryout fee, and fully completed tryout forms packet.

For dancers who only want to be considered for one of our Hip Hop teams, we do have a shortened version of tryout week and core classes/season membership packages. Please select the Hip Hop Only option when registering and pay the $100 tryout fee, and we'll reach out with specific information about the tryout week schedule as well as season requirements.

I understand that while my dancer's desired dance division(s) will be taken into serious consideration, the final decision regarding dance divisions for my dancer will be made by the WCAS coaches. I also understand that all placement decisions regarding team levels will be entirely made by WCAS coaches and are final. There is no guarantee that every potential team member trying out will receive a placement on a WCAS team for the 2024-25 season. *

RELEASE WAIVER

I certify that I am the parent or legal guardian for my child(ren).

I hereby give my permission for any supervisor, coach or other team administrator associated with FGZ Enterprises LLC dba World Class All Star Dance to seek and give appropriate medical attention for our child(ren) in the event of accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment. I hereby waive, release and forever discharge FGZ Enterprises LLC and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in FGZ Enterprises LLC activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my child(ren) is (are) physically fit and capable of participation in all FGZ Enterprises LLC activities and agree to not hold FGZ Enterprises LLC responsible for any injury, accident, or incident that may occur while my child(ren) are present in the facility.

By registering my child(ren) with FGZ Enterprises LLC, I agree to allow my child(ren) to participate in said event hosted by FGZ Enterprises LLC, and hereby release FGZ Enterprises LLC, its coaches, staff, and employees from liability for any injury that might occur to my child(ren) while participating in the FGZ Enterprises LLC program, including travel to and from activity, organization events or other scheduled activities.I agree to indemnify and hold harmless FGZ Enterprises LLC, their agents and/or employees, against any and all liability for personal injury, including injuries of any kind even resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) are participating in the FGZ Enterprises LLC program.

I also understand that FGZ Enterprises LLC reserves the right to cancel or change event schedule at any time for any reason deemed necessary. If course/event that I have paid for is changed or canceled for any reason and there is not an alternate option that fits my needs; I will be eligible for a full or partial refund. If I pay by credit card or ACH (online) I will be refunded the amount of the session or partial session and will not be refunded the processing fee. I will not be charged a refund processing fee if the refund is due to a change brought on directly from FGZ Enterprises LLC. I understand that if I decide to withdraw at my own will, FGZ Enterprises will decide if I am eligible for a refund.

I understand that FGZ Enterprises LLC produces promotional material about their program. I understand that my child may be included in video tape or photography taken during any activity sponsored by FGZ Enterprises LLC which may be published on any social and marketing content owned by FGZ Enterprises. I grant FGZ Enterprises LLC and all other commercial exhibitors the exclusive right to photograph and/or video tape my child and further grant the right to utilize their name, face, likeness, voice, and appearance as part of any sanctioned activity, advertising, or promotion without reservation or limitation. In granting this license, I understand that FGZ Enterprises LLC is under no obligation to exercise any of its rights, licenses, and privileges herein granted.
